We believe choice and control are important in the life of individuals with disabilities and their family. We support them in identifying and desired approach for service delivery and sort advice to assist in the best decision‐making processes.
Our Support Coordinators will help you connect with the supports you need as identified in your NDIS Support Plan, maintain your Plan by helping you source out best services in line with your recovery goals, and help you achieve your goals.
Assistance in coordinating or managing life stages, transition and supports include short and long-term supports that focuses on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community. These services include
- support connection
- coordination of supports (COS)
- Assistance with accommodation and tenancy obligations
- Life transition planning including mentoring, peer support and individual skill development.
- Assistance with decision making, daily planning, budgeting.
- Capacity building skills
- Development of independent self-care and living skills.
- Financial mentoring
- Establish supportive networks in a client’s community circle.
- Linkage support to other supportive community services and programs
